IV Therapy: What the Evidence Actually Shows

IV therapy has gone from hospital corridors to wellness menus in about a decade, and the marketing has run well ahead of the evidence. If you are considering a drip in West Palm Beach or Fort Myers, this is what it can reasonably do, what it cannot, who should not have one, and what we do before we place a line.

What IV therapy is

A small catheter is placed in a vein, usually in the arm, and a sterile solution of fluids with added vitamins, minerals or amino acids is infused over roughly 30 to 45 minutes. The formula is chosen for you after a review of your history. You sit, you are monitored, you leave.

Because the infusion goes directly into the bloodstream, it skips the digestive tract entirely. That is the real, physiologic difference between an IV and a supplement — and it is worth understanding precisely, because it is both smaller and larger than most clinics suggest.

The absorption question, answered honestly

The route genuinely matters for some nutrients. The NIH Office of Dietary Supplements notes that with oral vitamin C, absorption falls below 50% at doses above one gram per day, and even 3 grams every four hours produces plasma levels of only about 220 µmol/L — while intravenous administration can reach concentrations as high as 26,000 µmol/L. That is not a marketing claim; it is pharmacokinetics.

But higher is not automatically better. For vitamin B12, a 2018 Cochrane review found that high-dose oral B12 normalized blood levels about as well as intramuscular injection. And roughly 48% of Americans take in less magnesium than recommended from food, which is a real gap — but one a diet change also addresses.

So: the IV route reaches levels oral supplements cannot. Whether reaching those levels produces a health benefit in a person who is not deficient is a separate question, and the honest answer is that the evidence is thin.

What the research actually shows

The best-known wellness formula, the Myers’ cocktail, has been tested once in a randomized, double-blind, placebo-controlled trial — 34 patients with fibromyalgia, published in 2009. Both groups improved significantly from baseline, and there was no statistically significant difference between the infusion and placebo. The authors concluded that its efficacy “is as yet uncertain.”

The Merck Manual states plainly that there is insufficient scientific evidence that IV vitamin therapy is effective for treating any disease. Cleveland Clinic’s assessment is similar: limited evidence of a general wellness benefit, and “we’re still looking for high-quality studies.”

We would rather tell you that than sell you a promise. What we can say accurately: IV fluids do rehydrate you, the formulas contain nutrients your body uses in normal metabolic function, and many clients tell us they feel better afterward. That last part is a personal experience, not a clinical outcome.

What we do not claim

You will see other clinics advertise drips that “boost immunity,” “cure hangovers,” “detox” the body, or serve as adjuncts to serious disease. We do not make those claims, for two reasons.

The first is that they are not supported. On hangovers specifically, clinicians are direct: rehydrating with IV fluids will not cure a hangover, because dehydration is only one part of what a hangover is. On “detox,” there is no named toxin and no measurable endpoint.

The second is regulatory. The FTC requires competent and reliable scientific evidence — generally randomized controlled human trials — for health benefit claims, and it has explicitly stated that hedged wording like “may help” does not fix an unsupported claim. In 2018 the FTC brought its first enforcement action against an IV cocktail marketer for exactly this pattern of advertising. Any clinic still running “cancer adjunct” or “immune boost” drips is describing something regulators have already acted on.

Is IV therapy FDA approved?

No, and it is important to understand why. Custom nutrient infusions are compounded preparations. As the FDA states, compounded drugs are not FDA-approved, which means the agency does not verify their safety, effectiveness or quality before they are marketed.

That is precisely why where you get an IV matters. In 2021 the FDA specifically named IV hydration clinics, medical spas and mobile IV services in a warning about compounding under insanitary conditions, citing an outbreak in which 38 patients were exposed and 17 infected, and a case in which a woman was hospitalized with suspected septic shock after a vitamin infusion. In 2025, a Florida-manufactured NAD+ injection was recalled at the FDA’s most serious classification for elevated endotoxin levels.

Ask any provider where their solutions are compounded, whether they come from a licensed pharmacy, and who prepares them. If the answer is vague, leave.

Who should not have IV therapy

This is the part that gets skipped most often. IV therapy is not appropriate for everyone, and a good clinic screens before it infuses. Talk to a physician first — and expect us to ask about — the following:

  • Kidney disease or impaired renal function. The ability to clear excess magnesium falls, and high-dose vitamin C increases urinary oxalate. A 2020 scoping review in Critical Care Medicine covering 74 studies documented cases of oxalate nephropathy and kidney stones with high-dose IV vitamin C.
  • Heart failure, heart disease or uncontrolled high blood pressure — fluid volume is a real consideration.
  • G6PD deficiency. The same review documented three cases of hemolysis in G6PD-deficient patients receiving high-dose vitamin C, and recommended specific monitoring.
  • Pregnancy or breastfeeding. Safety has not been studied.
  • Abnormal magnesium or potassium levels, which can lead to arrhythmias or muscle weakness.
  • Current prescription medications, which we review for interactions.
